ABSTRACT

Introduction: Comorbidities in adults with Down syndrome are common making them a risk group for COVID-19. Thus, vaccinating against COVID-19 becomes necessary, but requires care against possible side effects. Objective: To investigate possible adverse effects after vaccination against COVID-19 in adults with Down Syndrome (DS). Methodology: Descriptive study in which ninety-seven adults diagnosed with DS were interviewed using a questionnaire containing seventeen questions related to personal and historical data, and seven questions related to vaccination. Results: The most applied vaccine was AstraZeneca (94%), Pfizer (4%), and CoronaVac (2%); 74% of the subjects had adverse effects, the most frequent after the first dose being pain at the injection site (65.3%), fever (37.9%), muscle pain (37.8%), malaise (29.5%); in the second dose there was a decrease in these adverse effects; 95% of symptoms disappeared after the first 24 hours following application. Conclusion: More than half of the individuals interviewed had adverse effects after the application of the first dose of the vaccine, decreasing with the second dose; the most frequent was pain at the site of application.


Introdução: Comorbidades em adultos com síndrome de Down são comuns tornando-os um grupo de risco para COVID-19. Dessa forma, vacinar contra a COVID-19 torna-se necessária, mas requer cuidados contra possíveis efeitos colaterais. Objetivo: Pesquisar possíveis efeitos adversos após vacinação contra COVID-19 em adultos com Síndrome de Down (SD). Metodologia: Estudo descritivo no qual 97 adultos diagnosticados com SD foram entrevistados por meio de um questionário contendo dezessete questões relacionadas a dados pessoais e histórico, e sete questões relacionadas à vacinação. Resultados: A vacina mais aplicada foi a AstraZeneca (94%), Pfizer (4%) e CoronaVac (2%); 74% dos sujeitos apresentaram efeitos adversos, sendo os mais frequentes após a primeira dose: dor no local da injeção (65,3%), febre (37,9%), dores musculares (37,8%), mal-estar (29,5%); na segunda dose houve diminuição desses efeitos adversos; 95% dos sintomas desapareceram após as primeiras 24 horas após a aplicação. Conclusão: Mais da metade dos indivíduos entrevistados apresentou efeitos adversos após a aplicação da primeira dose da vacina, diminuindo com a segunda dose; a mais frequente foi a dor no local de aplicação.

Resumo Introdução: A pandemia da COVID-19 contribuiu para a diminuição das atividades habituais dos idosos, o que pode interferir na capacidade funcional (CF) dos mesmos. Objetivo: Verificar o impacto da inatividade física durante a pandemia da COVID-19 na CF de idosos e associar o nível de atividade física com a CF dos mesmos. Métodos: Participaram do estudo 20 idosos que frequentavam um programa de atividade física antes da pandemia da COVID-19. Foi avaliada a capacidade funcional pelo teste AVD-Glittre, teste de caminhada de 6 minutos (TC6) e Time Up and Go (TUG); os resultados foram comparados com os resultados dos testes obtidos em 2019 (antes da pandemia da COVID-19). Para verificar o nível de atividade física, foi aplicado o Questionário Internacional de Nível de Atividade Física (IPAQ). Os dados foram analisados pelo teste t pareado de Wilcoxon e teste de correlação de Pearson. Resultados: Houve um aumento do tempo da execução do teste de AVD-Glittre de 171.90 ± 35.56 para 272.00 ± 344.77 segundos, no TUG teste de 6.27 ± 119 para 6.92 ± 1.53 segundos, e no TC6 houve uma diminuição de 564.97 ± 85.22 para 464.65 ± 105.95 metros, com p<0.001 em todas as análises. A correlação entre IPAQ e o teste de AVD-Glittre foi fraca (r=0.11, p<0.63), fraca com TC6 (r= -0.10, p>0.001) e moderada com TUG (r=0.59, p>0.001). Conclusão: Houve diminuição da CF dos idosos durante a pandemia da COVID-19, o nível de atividade física influenciou no TUG.


Introduction: The COVID-19 pandemic contributed to a decrease in the usual activities of older adults, which may interfere with their functional capacity (FC). Objective: To verify the impact of physical inactivity during the COVID-19 pandemic on the older adults' FC and associate the level of older adults' physical activity with the FC. Methods: The study included 20 older people who attended a physical activity program before the COVID-19 pandemic. Functional capacity was evaluated by the ADL-Glittre test, 6-minute walk test (6MWT) and Time Up and Go (TUG) test. The tests' results were compared to the tests obtained in 2019 (before the COVID-19 pandemic). The International Physical Activity Level Questionnaire (IPAQ) was applied to verify the physical activity level. Data were analyzed using Wilcoxon's paired t-test and Pearson's correlation test. Results: There was an increase in the execution time of the ADL-Glittre test from 171.90 ± 35.56 to 272.00 ± 344.77 seconds, in the TUG test from 6.27 ± 119 to 6.92 ± 1.53 seconds, and in the 6MWT there was a decrease from 564.97 ± 85.22 to 464.65 ± 105.95 meters, with p<0.001 in all analyses. The correlation between IPAQ and the ADL-Glittre test was weak (r=0.11, p<0.63), weak with 6MWT (r= -0.10, p>0.001) and moderate with TUG (r=0.59, p>0.001). Conclusion: There was a decrease in the FC of older adults during the COVID-19 pandemic; the level of physical activity influenced the TUG.

Introdução: Pacientes infectados com COVID-19 parecem manifestar evolução mais grave quando apresentam comorbidades, como a Diabetes Mellitus (DM). No entanto, dados da população brasileira ainda são pouco disponíveis. Objetivo: Analisar a associação entre DM e evolução para intubação e óbito em indivíduos internados com COVID-19. Metodologia: Estudo transversal, observacional, quantitativo com dados dos prontuários de 74 participantes adultos internados no Hospital Professora Lydia Storópoli, com diagnóstico de COVID-19. O Comitê de Ética em Pesquisa Humana aprovou este estudo e os participantes assinaram o termo de consentimento do estudo. Resultados: Dos 74 pacientes, 18 apresentavam DM, dos quais seis evoluíram para intubação e cinco evoluíram para óbito. Não houve associação significativa (p>0,05) entre DM e intubação e óbito. Conclusão: Na presente amostra a DM não foi associada com a evolução clínica para intubação e óbito.


Introduction: Patients infected with COVID-19 seem to manifest a more severe evolution when they have comorbidities, such as Diabetes Mellitus (DM). However, data on the Brazilian population are still scarcely available. Objective: To analyze the association between DM and evolution to intubation and death in individuals hospitalized with COVID-19. Methodology: Cross-sectional, observational, quantitative study with data from the medical records of 74 adult participants admitted to Professora Lydia Storópoli Hospital, diagnosed with COVID-19. The Human Research Ethics Committee approved this study, and participants signed the study consent form. Results: 74 patients with Covid-19, 18 had DM, of which six evolved to intubation, and five evolved to death. There was no significant association (p>0.05) between DM and intubation and death. Conclusion: In the present study, DM was not associated with clinical evolution to intubation and death.

Introduction: Adults with Down syndrome (DS) have functional disabilities due to the extra presence of chromosome 21. Objective: To identify the functionality and disability assessment instruments used in research involving adults with DS and associate them with the components of the International Classification of Functioning (ICF). Methods: Two independent researchers analyzed articles from PubMed, Lilacs, SciELO, Science Direct, and Cochrane databases, including cross sectional and clinical studies whose results involved functionality and disability for individuals with DS (≥18 years), with no publication date limit for the studies. The methodological quality of the studies was analyzed by the Downs & Black Checklist; descriptive analysis was used for the results. This review was registered in PROSPERO (CRD42021234012). Results: 15 articles were analyzed in which 48 instruments were identified for the assessment of adults with DS (36.42±10.62 years); the quality of the articles was considered "good". Of these 48 instruments, 41 were associated with bodily function, 5 instruments were associated with the activity component, one instrument was associated with social participation and one instrument was associated with the environment. Conclusion: Of the 48 instruments identified to assess adults with DS, most were for the Body Function and Structure component; only the 6MWT and CAMDEX-SD have been validated for this population. LIFE-H and MQE were used to assess Social Participation and the Environment, but they cannot be considered dependable, as they have not been confirmed for individuals with DS.

INTRODUCTION: The decline in functional capacity (FC) interferes with the functional independence of older adults, so it is important to assess the FC and use appropriate instruments for this. OBJECTIVE: To investigate the Glittre Activities of Daily Living (ADL) test's validity and reliability for assessing functional capacity in older adults. METHODS: Cross-sectional study with a sample of 100 elderly (68 ± 5.16 years). To assess the convergent validity, the Six-Minute Walk Test (6MWT) and the Timed Up and Go Test (TUG) were performed. The intra-examiner test-retest of the Glittre-ADL test was performed on the same day with a 30-minute interval between repetitions and inter-examiner reliability with an interval of seven days. RESULTS: There was a strong correlation between the Glittre-ADL test and the 6MWT (r=-0,75; p<0.001) and the TUG (r=0.77; p<0.001). The intra-examiner and inter-examiner reliability was excellent (ICC)=0.91 and 95% CI=0.14-0.97; p<0.001 and ICC=0.91; 95% CI: 0.86-0.94; p<0.001, respectively). CONCLUSION: The Glittre-ADL test demonstrated that it is valid and that its reliability is adequate to assess functional capacity in older adults.

ABSTRACT Objective: To analyze the quality of data collected during prenatal care recorded in the Integrated Health Care Management System (SIGA) of the Municipal Department of Health of São Paulo from 2012 to 2020. Methods: Descriptive study using SIGA data and the variables: maternal height (cm), weight (kg) measured throughout pregnancy, gestational age at prenatal consultation, systolic (SBP) and diastolic (DBP) blood pressure (in mmHg), and body mass index (BMI) at the beginning of pregnancy (up to 8 weeks). Quality analysis was carried out by calculating the indicators: percentage of incompleteness and zero values of all variables studied, percentage of implausible values for height, weight, BMI; preference for terminal digit of weight and height, and normality of distributions. Results: The database of pregnant women made available for analysis included 8,046,608 records and 1,174,115 women. The percentage of incompleteness and zero values was low (<1%) in all original variables of the system. There are more records at the end of pregnancy. For the four original variables of interest in the database (weight, height, SBP, DBP), there is a clear preference for the terminal digit. The variables of interest did not present an approximately normal distribution during the evaluated period. Conclusion: The quality analysis showed the need for improving the standardization of information collection and recording, the rounding of measurements and the need for encouraging pregnant women to start prenatal care as soon as possible, in such a way that it is important to invest in data quality, through educational resources for professionals who work in health care.


RESUMO Objetivo: Analisar a qualidade dos dados coletados no acompanhamento pré-natal registrados no Sistema Integrado de Gestão da Assistência à Saúde (SIGA) da Secretaria Municipal de Saúde de São Paulo de 2012 a 2020. Métodos: Estudo descritivo utilizando dados do SIGA e as variáveis: altura materna (cm), peso (kg) medido ao longo da gestação, idade gestacional na consulta pré-natal, pressão arterial (em mmHg) sistólica (PAS) e diastólica (PAD), e índice de massa corporal (IMC) no início da gestação (até 8 semanas). A análise da qualidade foi realizada por meio do cálculo dos indicadores: percentual de incompletude e valores zero de todas as variáveis estudadas, percentual de valores implausíveis de estatura, peso, IMC; preferência por dígito terminal do peso e estatura, e normalidade das distribuições. Resultados: Base de dados de gestantes disponibilizada para análise incluía 8.046.608 registros e 1.174.115 mulheres. O percentual de incompletude e valores zeros foi baixo (<1%) em todas as variáveis originais do sistema. Existe maior número de registros ao final da gestação. Para as quatro variáveis de interesse originais do banco de dados (peso, altura, PAS, PAD), existe clara preferência por dígito terminal. As variáveis de interesse não apresentaram distribuição aproximadamente normal durante o período avaliado. Conclusão: A análise da qualidade mostrou necessidade de melhoria na padronização da coleta e do registro das informações, no arredondamento das medidas e na necessidade de incentivar as gestantes a iniciar o pré-natal o quanto antes; por isso, é importante investir na qualidade do dado, por meio de recursos educativos para profissionais que atuam na assistência.

Abstract Introduction Glycemic control is important to avoid diabetes complications in individuals with cancer. There is no evidence for HbA1c and fructosamine as reliable biomarkers in these conditions. There are particularities in caring for patients with diabetes and cancer that can alter these biomarkers. Objective The aim of this study was to evaluate HbA1c and fructosamine as glycemic biomarkers in people with type 2 diabetes and cancer, undergoing clinical or surgical oncological treatment. Methods The authors conducted a single-center, retrospective analysis with people who have cancer and diabetes. Comparison of glycemic biomarkers (HbA1c, fructosamine, and Self-Monitoring of Blood Glucose [SMBG]) was performed including evaluation in individuals undergoing chemotherapy, using glucocorticoids, with anemia, hypoproteinemia or with reduced estimated Glomerular Filtration Rate (eGFR). Results There was a strong positive correlation between fructosamine and HbA1c (n = 318, r= 0.66, p < 0.001) in people with diabetes and cancer even in those under chemotherapy (n = 101, r= 0.61, p < 0.001) or using glucocorticoids (n = 96, r= 0.67, p<0.001). There was a strong correlation between HbA1c and fructosamine in subjects with anemia (n = 111, r= 0.66, p < 0.001), hypoproteinemia (n = 54, r= 0.67, p < 0.001), or with eGFR ≥ 60 mL/min/1.73 m2 (n = 189, r= 0.70, p < 0.001), and moderate correlation with hypoalbuminemia (n = 21, r= 0.54, p = 0.001) and with reduced eGFR (n = 67, r= 0.57, p < 0.001). The correlations between fructosamine and HbA1c with SMBG were moderate (n = 164, r= 0.49, p < 0.001; n = 111, r= 0.55, p < 0.001, respectively), strong in subjects undergoing chemotherapy, with hypoalbuminemia or hypoproteinemia, and at least moderate, if eGFR < 60 mL/min/1.73 m2 or with anemia. Conclusions Fructosamine and HbA1c can be used as glycemic biomarkers in people with diabetes and cancer, even in those with anemia, hypoproteinemia, or undergoing chemotherapy.

RESUMO O objetivo deste estudo é avaliar o efeito da estimulação elétrica transcutânea do nervo vago (EETNV) na inflamação, modulação autonômica cardíaca e evolução clínica dos pacientes com COVID-19. Trata-se de um ensaio clínico, controlado por sham, randomizado e cego, no qual participarão 52 indivíduos hospitalizados com diagnóstico de COVID-19, que serão randomizados em dois grupos de tratamento: grupo experimental (tratamento médico usual associado à EETNV ativa) ou grupo-controle (tratamento médico usual associado à EETNV sham). A EETNV será realizada por meio de um estimulador elétrico neuromuscular (modelo Dualpex 071 da Quark Medical Products), com o eletrodo de estimulação posicionado sobre o tragus esquerdo, com corrente alternada, frequência de 30Hz e variação de 50%, intensidade ajustada para o limiar sensorial do paciente, com duração de 90 minutos cada sessão de estimulação, duas vezes ao dia, durante sete dias consecutivos, totalizando 14 sessões. Serão avaliados pré e pós-intervenção o nível de interleucina-6 (IL-6) e interleucina-10 (IL-10), cortisol e proteína C reativa (PCR), pressão arterial, variabilidade da frequência cardíaca pelos parâmetros de baixa frequência (BF), alta frequência (AF) e pela razão da baixa e alta frequência (BF/AF), além de evolução clínica dos pacientes, incluindo o nível de ansiedade e depressão, cujos dados serão obtidos por meio de prontuários e questionários. Será realizado também um acompanhamento 7 e 14 dias após o término das intervenções para verificar a evolução clínica, incluindo nível de ansiedade e depressão, e durante seis meses serão avaliadas memória e atenção.


RESUMEN El objetivo de este estudio es evaluar el efecto de la estimulación eléctrica transcutánea del nervio vago (EETNV) sobre la inflamación, la modulación autonómica cardíaca y la evolución clínica de pacientes con COVID-19. Se trata de un ensayo clínico, controlado por simulado, aleatorizado y ciego, en el que participarán 52 individuos hospitalizados diagnosticados de COVID-19, que serán aleatorizados en dos grupos de tratamiento: grupo experimental (tratamiento médico habitual asociado a la EETNV activa) o grupo control (tratamiento médico habitual asociado a la EETNV simulada). La EETNV se realizará mediante un estimulador eléctrico neuromuscular (modelo Dualpex 071 de Quark Medical Products), con el electrodo de estimulación colocado en el trago izquierdo, con corriente alterna, frecuencia de 30Hz y 50% de variación, intensidad ajustada al umbral sensorial del paciente, con una duración de 90 minutos cada sesión de estimulación, dos veces al día, durante siete días consecutivos, lo que totaliza 14 sesiones. Se evaluarán antes y después de la intervención la interleucina-6 (IL-6) y la interleucina-10 (IL-10), el cortisol y la proteína C reactiva (PCR), la presión arterial, la variabilidad de la frecuencia cardíaca por los parámetros de baja frecuencia (BF), alta frecuencia (AF) y razón de baja y alta frecuencia (BF/AF), así como la evolución clínica de los pacientes, incluidos los parámetros de ansiedad y depresión cuyos datos se obtendrán de historias clínicas y cuestionarios. También se realizará un seguimiento de 7 y 14 días tras finalizadas las intervenciones para verificar la evolución clínica, incluidos el nivel de ansiedad y de depresión, y durante seis meses se evaluará la memoria y la atención.


ABSTRACT This study aims to evaluate the effect of transcutaneous auricular vagus nerve stimulation (taVNS) on inflammation, cardiac autonomic modulation, and clinical evolution of patients with COVID-19. This is a clinical, sham-controlled, randomized, and blind trial, in which 52 hospitalized individuals diagnosed with COVID-19 will participate. They will be randomized into: experimental group (usual medical treatment associated with active taVNS) and control group (usual medical treatment associated with sham taVNS). The taVNS will be performed by a neuromuscular electric stimulator (Dualpex model 071 of Quark Medical Products), with the stimulation electrode positioned on the left tragus, with alternating current, at a 30Hz frequency with 50% variation. Intensity will be adjusted to the patient's sensory threshold, with 90-minutes-long stimulation sessions, happening twice per day for seven consecutive days, totaling 14 sessions. Interleukin-6 (IL-6) and interleukin-10 (IL-10), cortisol and C-reactive protein (CRP), blood pressure, heart rate variability (HRV) by low frequency (LF), high frequency (HF) and low and high frequency ratio (LF/HF) parameters will be evaluated before and after the intervention, as well as patients' clinical evolution-including anxiety and depression levels-whose data will be obtained through medical records and questionnaires. A follow-up will also be performed seven and 14 days after the end of the interventions to verify the clinical evolution, including anxiety and depression levels. Memory and attention levels will be evaluated for six months.

ABSTRACT Objective: Analyze the temporal trend and spatial distribution of the incidence rate of dengue cases in Paraná and its regions between 2012 to 2021 and investigate associated sociodemographic and environmental variables. Methods: Ecological study with temporal and spatial analysis of the dengue incidence rate reported in the Disease and Notification Information System (SINAN) in the period 2012 to 2021 and investigation of sociodemographic and environmental variables. To identify differences between municipal incidence rates the Mann-Whitney and Kruskal-Wallis test followed by Dunn's test for multiple comparisons were used. Prais-Winsten regression was used for temporal trend analysis and for spatial analysis the univariate and bivariate Local Moran analysis were applied. Results: 548,683 cases of dengue were confirmed in the period, the highest state incidence rate was observed in 2020, with 15 health regions presenting more than 500 cases/100,000 inhabitants. Higher incidences were observed among women, age group of 20-59 years and white color/race. Despite annual variations, a stationary trend was observed for incidence rates according to sex, age group, color/race and macro-region. More than half of the municipalities in Paraná formed spatial clusters (Moran's I=0.679), where 73 (18.3%) municipalities with high incidence rate formed clusters. High-High clusters of dengue incidence rate with urbanization and High-Low clusters of incidence rate with vegetation cover were observed. Conclusion: Sociodemographic and environmental determinants were related to the high incidence rates of dengue and heterogeneous spatial distribution in the state of Paraná, indicating the need to strengthen health surveillance actions.


RESUMO Objetivo: Analisar a tendência temporal e a distribuição espacial da taxa de incidência de casos de dengue no Paraná e suas regiões entre 2012 e 2021 e investigar variáveis sociodemográficas e ambientais associadas. Métodos: Estudo ecológico com análises temporais e espaciais da taxa de incidência da dengue registrada no Sistema de Informação de Agravos de Notificação, de 2012 e 2021, e investigação de variáveis sociodemográficas e ambientais. Para analisar as taxas de incidência municipais foram utilizados os testes de Mann-Whitney e Kruskal-Wallis, seguidos do teste de Dunn para múltiplas comparações. Utilizou-se para análise da tendência temporal a regressão de Prais-Winsten, e para a análise espacial, o índice de Moran Local univariado e bivariado. Resultados: Foram confirmados 548.683 casos de dengue no período, apresentando a maior taxa de incidência estadual em 2020, com 15 regionais de saúde registrando mais de 500 casos/100 mil habitantes. Maiores incidências ficaram entre mulheres, faixa etária de 20-59 anos e cor/raça branca. Apesar de variações anuais, observou-se tendência estacionária para incidência segundo sexo, faixa etária, cor e macrorregião. Mais da metade dos municípios paranaenses formou aglomerados espaciais (Moran's 1=0,679) — 73 (18,3%) municípios com alta taxa de incidência formaram agrupamentos. Foram identificados agrupamentos da taxa de incidência da dengue com o grau de urbanização (alto-alto) e com o percentual de cobertura vegetal natural (alto-baixa). Conclusão: Determinantes sociodemográficos e ambientais relacionaram-se com as altas taxas de incidência da dengue e com a distribuição espacial heterogênea no estado do Paraná, indicando a necessidade do fortalecimento das ações de vigilância em saúde.

ABSTRACT The fall armyworm, Spodoptera frugiperda (J. E. Smith) (Lepidoptera: Noctuidae), is an important rice pest which consumes leaf area during all phases of plant development. The objective of this study was to identify rice genotypes that provide antixenosis and antibiosis to fall armyworm. Antixenosis was investigated by measuring non-preference in free-choice and no-choice tests while antibiosis was determined by measuring biological parameters and nutritional indices of S. frugiperda in twelve rice genotypes. The genotype Miúdo Branco showed antixenosis to S. frugiperda, whereas the genotypes IR 64 and Bacaba Branco showed antibiosis to S. frugiperda by affecting insect development and prolonging insect life cycle. Insects that fed on Bacaba Branco genotype demonstrated the lowest nutritional indices. Since Miúdo Branco, IR 64 and Bacaba Branco showed moderate resistance, rice producers could use these genotypes as part of a control strategy for S. frugiperda.

ABSTRACT

Introduction Individuals with pre-diabetes have altered glycemic levels, are generally asymptomatic, and are at increased risk for developing type 2 diabetes mellitus. Objective Identify the prevalence of periodontal individuals with undiagnosed hyperglycemia and associated impact factors. Material and method Fifty-six patients with periodontitis and without diabetes self-report, users of dental clinic services at Federal University of Juiz de Fora were included in this research, during one year and a half of experimental evaluation. Socioeconomic and demographic data, anthropometric patterns, fasting capillary blood glucose, and complete periodontal examination (six sites per tooth) were evaluated. Result The sample consisted of 58.9% female, mean age 53 years old, 58.9% obese/overweight and 45.3% had a low level of education. A total of 28.6% (n=16) participants had undiagnosed hyperglycemia (between 100 to 160 mm / dL), of which 81.3% were obese/overweight, 25% were smokers, 56.3% reported having a history of diabetes in the family, 93.8% had a family income up to 2 brazilian´s minimum wages. BMI values ​​were higher in the group of patients with hyperglycemia (29.8 ± 5.7, p = 0.03) compared to the group without hyperglycemia (26.6 ± 5.6). Patients with hyperglycemia had a greater number of sites with clinical attachment loss (CAL) between 4 and 6 mm (p = 0.04) when compared with the normoglycemic group. Conclusion Undiagnosed CAL attachment loss between 4 and 6 mm due to periodontitis than normoglycemic individuals.


Introdução Indivíduos com pré-diabetes apresentam níveis glicêmicos alterados, geralmente são assintomáticos e apresentam risco aumentado para desenvolver diabetes mellitus tipo 2. Objetivo Identificar a prevalência de indivíduos periodontais com hiperglicemia não diagnosticada e os fatores de impacto associados. Material e método Cinquenta e seis pacientes com periodontite e sem autorrelato de diabetes, usuários de serviços de clínica odontológica da Universidade Federal de Juiz de Fora foram incluídos nesta pesquisa, durante um ano e meio de avaliação experimental. Foram avaliados dados socioeconômicos e demográficos, padrões antropométricos, glicemia capilar de jejum e exame periodontal completo (seis sítios por dente). Resultado A amostra foi composta por 58,9% do sexo feminino, média de idade de 53 anos, 58,9% obesidade / sobrepeso e 45,3% com baixa escolaridade. Um total de 28,6% (n=16) participantes tinham hiperglicemia não diagnosticada (entre 100 a 160 mm / dL), dos quais 81,3% eram obesos / com sobrepeso, 25% eram fumantes, 56,3% relataram ter histórico de diabetes na família, 93,8% tinham renda familiar de até 2 salários mínimos brasileiros. Os valores de IMC foram maiores no grupo de pacientes com hiperglicemia (29,8 ± 5,7, p = 0,03) em comparação ao grupo sem hiperglicemia (26,6 ± 5,6). Pacientes com hiperglicemia apresentaram maior número de sítios com perda clínica de inserção (CAL) entre 4 e 6 mm (p = 0,04) quando comparados ao grupo normoglicêmico. Conclusão A perda de inserção de CAL não diagnosticada entre 4 e 6 mm devido à periodontite do que indivíduos normoglicêmicos.

ABSTRACT

Este artigo busca analisar o aspecto mais cruel do fenômeno Bolsonaro, legitimado pela população: a indiferença ao genocídio impetrado à população mais pobre do país, por meio ao descaso do governo em tomar medidas que pudessem atenuar a pandemia. Por meio da análise da desilusão que essa indiferença revelou, tenta-se mostrar as três figuras do ressentimento manifestas no apoio e nas ações do governante: a de um severo supereu moralista; a da psicologia das massas, isto é, do ódio ao diferente, acirrado pelo racismo; e, por fim, da submissão incondicional e auto-sacrifício. Três figuras constituintes da psicologia dos filhos da horda primitiva, descritas por Freud em Visão geral das neuroses de transferência. A questão principal abordada no artigo, no entanto, é a relação da indiferença ao genocídio e o racismo brasileiro, compreendido em sua dimensão estrutural, histórica e atual.


This article seeks to analyze the most cruel aspect of the Bolsonaro phenomenon, legitimized by the population: indifference to genocide imposed on the poorest population in the country, through the government's neglect to take steps that could mitigate the pandemic. Through the analysis of the disillusion that this indifference revealed, we try to show the three figures of resentment manifested in the government's support and actions: a severe moralist superego; the psychology of the masses, that is, the hatred of different, intensified by racism; and, finally, unconditional submission and self-sacrifice. Three constituent figures of the children's psychology on the primitive horde, described by Freud in Overview of transference neuroses. The main issue addressed in the article, however, is the relationship of indifference to genocide and Brazilian racism, understood in its structural, historical and current dimension.

ABSTRACT

A violência sexual contra crianças e adolescentes é um grave problema no Brasil. Estratégias precisam ser planejadas visando sua prevenção e o cuidado das vítimas. O presente estudo objetivou compreender as ações de uma delegacia de proteção à criança e ao adolescente (DPCA) frente a casos de violência sexual. Para tal, foi realizado um estudo qualitativo por meio da inserção ecológica e de entrevistas com profissionais. Foram realizadas 13 observações do cotidiano da delegacia e seis entrevistas com profissionais. Os dados provenientes dos registros das observações e das entrevistas foram analisados por meio da análise temática. A análise dos dados resultou em três temas: funcionamento, recursos e emoções. A DPCA estava em um processo de transição devido à aprovação da Lei 13.431/17, acarretando em suspensão de algumas atividades. A suspensão dessas atividades, aliada à falta de recursos materiais e humanos, resultou em não cumprimento do papel protetivo e de promotor de desenvolvimento da DPCA. Embora engajados, os profissionais relataram dificuldades, inclusive emocionais, em atender à demanda da violência sexual. Dada a recenticidade da aprovação da Lei, talvez esse seja o primeiro estudo a evidenciar o processo de transição que um serviço enfrenta para adequar-se a ela. (AU)


Sexual violence against children and adolescents is a serious problem in Brazil. Strategies need to be planned aiming prevention and care of the victims. The present study aims to understand the actions taken by a police station to protect children and adolescents (DPCA, Portuguese acronym) in cases of sexual violence. A qualitative study was carried out, adopting the ecological insertion methodology and interviews. Thirteen observations of the daily life of the police station and six interviews with practitioners were carried out. Thematic analysis was adopted to analyze data from the observations and interviews, resulting in three themes: functioning, resources and emotions. The DPCA was in a process of transition due to the approval of Law 13.431/17, resulting in suspension of some activities. The suspension of these activities, coupled with the lack of material and human resources, resulted in non-compliance with the roles of the DPCA. Although engaged, professionals reported difficulties, including emotional difficulties, in meeting the demand for sexual violence. Given the recent approval of Law, perhaps this is the first study to evidence the process of transition that a service faces to suit it. (AU)

Se ha objetivado comprender las acciones de una Comisaría de Protección al Niño y al Adolescente (DPCA, sigla en portugués) frente a casos de violencia sexual. Se realizó un estudio cualitativo por medio de la inserción ecológica y de entrevistas con profesionales. Se realizaron trece observaciones del cotidiano de la comisaría y seis entrevistas con profesionales. Los datos fueron analizados por medio del análisis temático. Se han identificado tres temas: funcionamiento, recursos y emociones. La DPCA estaba en un proceso de transición debido a la aprobación de la Ley 13.431/17, conllevando la suspensión de algunas actividades. La suspensión de esas actividades, asociada a la falta de recursos materiales y humanos, resultó en el no cumplimiento del papel protector y de promotor de desarrollo de la DPCA. Aunque comprometidos, los profesionales relataron dificultades, incluso emocionales, de atender a la demanda de la violencia sexual. Dada el carácter reciente de la aprobación de la Ley, tal vez ese sea el primer estudio que demuestre el proceso de transición que un servicio enfrenta para adecuarse a ella. (AU)

ABSTRACT The somatotropic axis is the main hormonal regulator of growth. Growth hormone (GH), also known as somatotropin, and insulin-like growth factor 1 (IGF-1) are the key components of the somatotropic axis. This axis has been studied for a long time and the knowledge of how some molecules could promote or impair hormones production and action has been growing over the last decade. The enhancement of large-scale sequencing techniques has expanded the spectrum of known genes and several other candidate genes that could affect the GH-IGF1-bone pathway. To date, defects in more than forty genes were associated with an impairment of the somatotropic axis. These defects can affect from the secretion of GH to the bioavailability and action of IGF-1. Affected patients present a large heterogeneous group of conditions associated with growth retardation. ABSTRACT The first description of patients with combined pituitary hormone deficiencies (CPHD) caused by PROP1 mutations was made 20 years ago. Here we updated the clinical and genetic characteristics of patients with PROP1 mutations and summarized the phenotypes of 14 patients with 7 different pathogenic PROP1 mutations followed at the Hospital das Clínicas of the University of Sao Paulo. In addition to deficiencies in GH, TSH, PRL and gonadotropins some patients develop late ACTH deficiency. Therefore, patients with PROP1 mutations require permanent surveillance. On magnetic resonance imaging, the pituitary stalk is normal, and the posterior lobe is in the normal position. The anterior lobe in patients with PROP1 mutations is usually hypoplastic but may be normal or even enlarged. Bi-allelic PROP1 mutations are currently the most frequently recognized genetic cause of CPHD worldwide. PROP1 defects occur more frequently among offspring of consanguineous parents and familial cases, but they also occur in sporadic cases, especially in countries in which the prevalence of PROP1 mutations is relatively high. We classified all reported PROP1 variants described to date according to the American College of Medical Genetics and Genomics and the Association for Molecular Pathology (ACMG-AMP) guidelines: 29 were pathogenic, 2 were likely pathogenic, and 2 were of unknown significance. An expansion of the phenotype of patients with PROP1 mutations was observed since the first description 20 years ago: variable anterior pituitary size, different pathogenic mutations, and late development of ACTH deficiency. ABSTRACT

Objective: to verify the accuracy of cone beam computedtomography (CBCT) for measuring dentin thicknessin the danger zone of mandibular molars in order to validatethis method to be used as a clinical auxiliary. Materialsand method: dentin thickness of the distal wall ofmesial canals was measured at 2 mm of furcation areain ten mandibular molars before and after preparation ofcervical and middle thirds. CBCT (0.25 mm voxel size)and stereomicroscopy images were acquired twice foreach sample. CBCT axial images and stereomicroscopeimages were evaluated by a calibrated examiner. Statisticalanalysis was performed using Mann-Whitney testin order to investigate whether CBCT images providedtrusted values. Results: mean values of initial (1.23 versus1.24 mm) and final measurements (0.88 versus 0.87mm) obtained with, respectively, CBCT and stereomicroscopemethods did not show statistical differences(P > 0.5). Conclusion: CBCT images provide high accuracyfor measuring dentin thickness in the danger zoneof mandibular molars. (AU)


Objetivo: avaliar a acurácia da tomografia computadorizada de feixe cônico (TCFC) para mensurar a espessura dentinária na zona de risco de molares inferiores e validar este método, a fim de usá-lo como um auxiliar clínico. Materiais e método: a espessura dentinária da parede distal dos canais mesiais foi medida a 2 mm da área de furca em dez molares inferiores, antes e após a preparação dos terços médio e cervical. Foram adquiridas imagens por TCFC (tamanho do voxel de 0,25 mm) e estereomicroscopia. Um examinador treinado e calibrado avaliou as tomografias no plano axial e as imagens de estereomicroscopia. A análise estatística foi realizada com o teste Mann-Whitney, para verificar se as imagens tomográficas forneceram valores comparáveis à estereomicroscopia. Resultados: a média dos valores obtidos, respectivamente, após as mensurações iniciais (1,23 versus 1,24 mm) e finais (0,88 versus 0,87 mm) das imagens de TCFC e estereomicroscopia não mostraram diferença estatisticamente significativa entre os métodos (P > 0,5). Conclusão: a TCFC produz imagens acuradas para a mensuração da espessura dentinária na zona de risco de molares inferiores. (AU)

SUMMARY Isolated growth hormone deficiency (IGHD) is the most common pituitary hormone deficiency and, clinically, patients have delayed bone age. High sequence similarity between CYP21A2 gene and CYP21A1P pseudogene poses difficulties for exome sequencing interpretation. A 7.5 year-old boy born to second-degree cousins presented with severe short stature (height SDS −3.7) and bone age of 6 years. Clonidine and combined pituitary stimulation tests revealed GH deficiency. Pituitary MRI was normal. The patient was successfully treated with rGH. Surprisingly, at 10.8 years, his bone age had advanced to 13 years, but physical exam, LH and testosterone levels remained prepubertal. An ACTH stimulation test disclosed a non-classic congenital adrenal hyperplasia due to 21-hydroxylase deficiency explaining the bone age advancement and, therefore, treatment with cortisone acetate was added. The genetic diagnosis of a homozygous mutation in GHRHR (p.Leu144His), a homozygous CYP21A2 mutation (p.Val282Leu) and CYP21A1P pseudogene duplication was established by Sanger sequencing, MLPA and whole-exome sequencing. We report the unusual clinical presentation of a patient born to consanguineous parents with two recessive endocrine diseases: non-classic congenital adrenal hyperplasia modifying the classical GH deficiency phenotype. We used a method of paired read mapping aided by neighbouring mis-matches to overcome the challenges of exome-sequencing in the presence of a pseudogene.

OBJETIVO: Este artigo tem por objetivo relatar as possíveis vantagens de curativos na forma de lâminas impregnadas com prata iônica (Atrauman®, Mepilex border Ag®, Mepilex-Ag® e Silvercel não aderente®) como agentes tópicos substitutos do creme de sulfadiazina de prata 1% nas queimaduras profundas. RELATO DOS CASOS: Foram tratados 31 pacientes. O Atrauman-Ag® foi empregado em 15 pacientes, o Mepilex border Ag® em três, Mepilex-Ag® em quatro e o Silvercel não aderente® em nove pacientes. Do total, relatamos a utilização destes novos curativos em dois pacientes do estudo com diagnóstico inicial de queimadura profunda. Em nenhum caso foram observados sinais de infecção nas feridas, apesar das trocas de curativos terem sido realizadas entre 4 a 7 dias, mesmo nas queimaduras de espessura total. O conforto propiciado por estes curativos foi evidente, minimizando o estresse e dor e as feridas de espessura parcial se apresentaram com sinais evidentes de rápida epitelização. CONCLUSÃO: Neste estudo preliminar, notamos que todos pacientes se beneficiaram de modo significativo do uso de curativos modernos com lâminas impregnadas por prata iônica quando comparados ao curativo convencional de sulfadiazina. Além da proteção contra infecção, mesmo nas queimaduras de espessura total, eles oferecem a enorme vantagem de as trocas serem espaçadas entre 4 a 7 dias, quando comparados à sulfadiazina, que exige a troca diária dos curativos, minimizando dor e desconforto aos pacientes e também o estresse da equipe de saúde envolvida nos Centros de Tratamento de Queimados.(AU)


OBJECTIVES: This article aims to evaluate the efficacy, and possible advantages of dressings in the form of ionic silver impregnated sheets (Atrauman Ag®, Mepilex Border Ag®, Mepilex - Ag® and Silvercel non - adherent®) as a local substitute agents for the cream of silver sulfadiazine-1% on deep burns. CASE REPORTS: 31 patients were treated. Atrauman-Ag® was used in 15 patients, Mepilex Border Ag® in three, Mepilex-Ag® in four and Silvercel non-adherent® in nine patients. Of the total, we report the use of these new dressings in two patients. In no case were signs of wound infection, even though dressing changes were performed between 4 and 7 days, also in total thickness burns. The comfort provided by these dressings was evident, minimizing stress and pain and partial thickness wounds presented with clear signs of rapid epithelization. CONCLUSION: In this preliminary study, we noticed that all patients benefited significantly from the use of new dressings with ionic silver impregnated sheets when compared to the conventional dressing of sulfadiazine cream. In addition to protection against infection, even in full-thickness burns, they offer the significant advantage that the changes of dressings are spaced between 4 and 7 days. Compared to sulfadiazine, which requires the daily exchange of dressings, they minimize pain and discomfort to patients, and also the stress to the health team involved in the treatment of the Burn Treatment Centers.(AU)


Objetivos: Este artículo tiene como objetivo evaluar la eficacia preliminar y los beneficios potenciales de la curación en forma de láminas impregnadas con plata iónica (Atrauman®, Mepilex Border Ag®, Mepilex-Ag® y Silvercel no aderente®) como sustitutos de crema de sulfadiazina de plata al 1% en quemaduras profundas. Casos Clínicos: Se han tratado 31 pacientes. El Atrauman-Ag® se utilizó en 15 pacientes, Mepilex Border Ag® en tres, Mepilex Ag® en cuatro y Silvercel no adherente® en nueve pacientes. Del total, se presenta la utilización de estos nuevos apósitos en dos pacientes. En ningún caso se observaron signos de infección en las heridas, a pesar de los cambios de apósito se llevaron a cabo entre los 4 y 7 días, incluso en quemaduras de espesor total. La comodidad ofrecida por éstos curación era evidente, lo que minimiza el estrés y el dolor y las heridas de espesor parcial se presentan con signos claros de una rápida epitelización. Conclusión: En este estudio preliminar, se observó que todos los pacientes se beneficiaron significativamente del uso de apósitos impregnados con plata iónica en comparación con sulfadiazina. Además de la protección contra la infección, incluso en quemaduras de espesor total, ofrecen la gran ventaja de las cambios están separadas de 4 a 7 días en comparación con sulfadiazina, que requiere el cambio diario de vendajes, minimizando dolor y malestar a los pacientes, e también el estrés del personal de salud de los centros de tratamiento de Queimados.(AU)

ABSTRACT

O hemangioma infantil é um tumor vascular benigno que ocorre devido a uma proliferação anormal dos vasos sanguíneos. O quadro clínico apresenta três fases bem definidas: proliferativa, involutiva e involuída. O diagnóstico é realizado basicamente por meio da anamnese e do exame físico, e quando necessário preconiza-se avaliação histopatológica. O presente trabalho, descreve um caso clínico de um hemangioma presente em um bebê de 3 meses de idade que foi, de principio, diagnosticado como mucocele ou fibroma. A cirurgia excisional foi realizada eo material encaminhado para análise histopatológica, confirmando o diagnóstico de hemangioma. Nessas situações, vale ressaltar a importância do diagnostico diferencial, manobra cirúrgica adequada e a avaliação das características clínicas da lesão para evitar possíveis complicações cirurgicas.


The infantile hemangioma is a benign vascular tumor which occurs due to an abnormal proliferation of blood vessels. The clinical features three well-defined phases: proliferative, involution,and involuted. The diagnosis is made primarily by clinical history and physical examination, but when necessary, help to close the histopathological diagnosis. This paper describes a clinical case of a gift hemangioma in a baby three months old who was, in principle, diagnosed as mucocele or fibroma. The excisional surgery was performed and material sent for histopathological confirmation hemangioma. It is worth emphasizing the importance of differential diagnosis, appropriate surgical maneuver, assessment of clinical characteristics of the lesion to prevent potential surgical complications possible.

ABSTRACT

Objective: To analyze the marginal adaptation of two different Glass Ionomer Cements (GIC) after erosive challenges. Material and Methods: Sixty sound extracted primary canines were selected and class V cavities were made. Teeth were allocated into 6 groups according to restorative material: 1) high-viscosity GIC (Ketac™Molar Easymix) and 2) resin-modified GIC with nanoparticles (Ketac™N100), low pH beverage erosive challenge (Orange juice and Coca-Cola) or distilled water as control. Thereafter the sample was restored and subjecting to thermocycling. The sample was immersed for a 10-days period for the erosive simulation and then embedded in methylene blue solution during 4 hours. Finally teeth were sectioned for further analysis. Marginal adaptation test was performed by two trained examiners using the Salama et al. criteria. Descriptive and Kruskal-Wallis test (α=5%) were used to analyze the data. Results: The groups treated with Ketac™Molar EasyMix were similar in terms of marginal sealing ability when submitted to Orange juice and Coca-Cola but significantly worse than water. For samples restored with the Ketac™N100 the worst results were found in the Coca-Cola group. Conclusion: Erosive challenges with acidic drinks affected the marginal adaptation of the tested GIC.
